Privacy Statement

Dyslexia Coventry

This Privacy Statement explains how Dyslexia Coventry collects, uses, stores and protects personal information in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018.

  

1. Who We Are

Dyslexia Coventry is an independent specialist assessment and consultancy service led by Sarah Grainger, Specialist Teacher Assessor (SpLD).

Dyslexia Coventry provides educational assessment, consultancy and support services for children, young people and adults.

Dyslexia Coventry is the data controller for the personal information collected and processed in relation to commissioned services.

  

2. What Information We Collect

Dyslexia Coventry may collect and process the following information:


  • Names, addresses and contact details 
  • Dates of birth 
  • Educational history and attainment information 
  • SEND and additional needs information 
  • Medical or developmental background information relevant to assessment 
  • Assessment scores and professional observations 
  • Reports and recommendations 
  • School, college, university or employer information 
  • Payment and invoicing information 
  • Communication records, including emails, telephone calls and voice notes 
  • Previous assessment reports, screeners, educational reports or supporting      documentation provided by families, schools or organisations 
  • Safeguarding information where relevant 

Information may be collected directly from clients, parents, carers, adult learners, schools, colleges, local authorities, employers or other professionals involved in supporting the learner.

  

3. How Information Is Collected

Information may be collected through:


  • Email correspondence 
  • Telephone calls 
  • Voice notes 
  • Questionnaires and information forms 
  • Google Forms 
  • In-person meetings and assessments 
  • Reports and documentation provided by schools, colleges, employers or families 
  • Educational consultations and follow-up discussions 

Dyslexia Coventry may also receive copies of previous reports, literacy assessments, dyslexia screeners or related educational documentation where relevant to the commissioned work.

  

4. Why We Collect Information and Our Lawful Basis

Dyslexia Coventry collects information in order to:


  • Arrange and deliver assessments and consultancy services 
  • Produce professional reports and recommendations 
  • Understand educational history and areas of need 
  • Communicate with clients regarding commissioned services 
  • Maintain professional records 
  • Meet safeguarding and legal obligations 
  • Fulfil APC renewal, moderation and professional quality assurance requirements


The lawful bases relied upon under UK GDPR may include:


  • Consent      
  • Performance of a contract 
  • Legitimate interests 
  • Legal obligations 
  • Safeguarding and protection obligations where applicable 


Some information processed by Dyslexia Coventry may constitute “special category data” under UK GDPR, including educational, developmental or health-related information relevant to assessment and support.

  

5. How Information Is Used

Information is used only for purposes connected to the commissioned service and professional practice.


Reports are produced using information gathered through background questionnaires, assessment data, professional observations and supporting documentation.


Completed reports are sent directly to the fee-paying client only.


The fee-paying client may be:


  • A parent or carer 
  • An adult learner 
  • A school, college or university 
  • A local authority 
  • An employer or organisation commissioning the work 


Dyslexia Coventry does not ordinarily distribute reports directly to third parties such as schools, GP services, tutors or employers on behalf of clients unless legally required to do so.

Clients may choose to share reports themselves with relevant professionals or organisations.

Dyslexia Coventry may occasionally send professional information, service updates or marketing communications to schools, educational settings, local authorities and organisations regarding available services. Such communications do not contain confidential client information.

Any testimonials or examples used for professional or promotional purposes will be anonymised unless explicit consent has been obtained.

  

6. How Long Information Is Kept

Assessment records and reports are retained securely in accordance with legal, safeguarding and professional requirements.


For children and young people, records may be retained until the learner reaches 25 years of age.

Adult records are ordinarily retained for a minimum of seven years from the date of assessment.

Following the retention period, records will be securely deleted, destroyed or disposed of unless there is a legal or professional reason requiring continued retention.

  

7. Who Information May Be Shared With

Information will only be shared where:


  • Explicit permission has been provided by the fee-paying client or adult learner 
  • There is a safeguarding concern or legal obligation requiring disclosure 
  • Disclosure is required for professional regulation, APC renewal, moderation or quality assurance purposes 


Where reports are used for APC renewal, moderation or quality assurance purposes, identifying information will be anonymised wherever reasonably possible.

Dyslexia Coventry does not sell personal information to third parties.

  

8. How Information Is Kept Safe

Dyslexia Coventry takes reasonable and proportionate steps to protect personal information.

Information may be stored using:


  • Password-protected computers and devices 
  • Password-protected cloud storage systems, including Google Drive 
  • Secure external hard drives 
  • Secure email systems 


Reasonable measures are taken to reduce the risk of unauthorised access, disclosure, loss or misuse of information.


While every effort is made to protect information, no electronic method of communication or storage can be guaranteed to be completely secure.

  

9. Your Rights

Under UK GDPR, individuals may have rights including:


  • The right to access personal information 
  • The right to request correction of inaccurate information 
  • The right to request restriction of processing in certain circumstances 
  • The right to object to processing in certain circumstances 
  • The right to request deletion of information where legally appropriate 
  • The right to lodge a complaint with the Information Commissioner’s Office (ICO) 


Requests relating to personal information should be made in writing via email.

  

10. Safeguarding and Legal Disclosure

Dyslexia Coventry may disclose information without consent where there is:


  • A safeguarding concern 
  • A risk of harm to an individual 
  • A legal obligation requiring disclosure 


Any such disclosure will be made in accordance with safeguarding, legal and professional responsibilities.

  

11. Parental Responsibility and Consent

Where information is provided by a parent, carer or individual with parental responsibility, Dyslexia Coventry will reasonably assume that all individuals with parental responsibility have been appropriately consulted and consent to:


  • The assessment proceeding 
  • Information being provided for assessment purposes 
  • Relevant communication regarding the commissioned service 

  

12. Website and Cookies

The Dyslexia Coventry website may use essential website cookies and basic website analytics provided through the website hosting platform.


These are used to support website functionality, security and general website performance monitoring.


Dyslexia Coventry does not knowingly use website tracking for the purpose of selling personal data.
