Terms and Conditions


Dyslexia Coventry


Effective Date: May 2026
Review Date: May 2027


These Terms and Conditions form part of the agreement between Dyslexia Coventry and the commissioning client.


These Terms and Conditions apply to all services commissioned through Dyslexia Coventry and delivered by Sarah Grainger, Specialist Teacher Assessor (SpLD), unless otherwise agreed in writing.

  

1. Audience and Purpose

These Terms and Conditions apply to all clients commissioning services through Dyslexia Coventry, including but not limited to:


  • Parents and carers 
  • Adult learners 
  • Schools, colleges and universities 
  • Local authorities 
  • Adult education providers 
  • Employers and workplace services 
  • Other organisations commissioning assessment, consultancy or training services 

The purpose of these Terms and Conditions is to provide clear information regarding the services offered, booking arrangements, fees, confidentiality, data protection, professional responsibilities and complaints procedures.


By confirming and proceeding with a booking, the client confirms that they have read, understood and accepted these Terms and Conditions.

  

2. Scope of Services

Dyslexia Coventry provides specialist educational assessment, consultancy and support services, which may include, but are not limited to:


  • Full diagnostic dyslexia assessments 
  • Dyslexia update assessments 
  • Literacy assessments and screeners 
  • Exam access arrangements assessments 
  • Workplace assessments and recommendations 
  • Educational consultancy and advisory services 
  • Educational advocacy and liaison 
  • Training, presentations and CPD 
  • Written reports and recommendations 
  • Follow-up consultations and support 


Services are delivered based upon the information available at the time of assessment or consultation and are carried out in accordance with professional standards applicable to Specialist Teacher Assessors.


Current fees are published on the Dyslexia Coventry website and may be updated periodically without prior notice.

  

3. Assessor’s Responsibilities

Sarah Grainger will:


  • Deliver services professionally, ethically and within the scope of professional competence 
  • Hold appropriate professional qualifications, memberships and certification relevant to specialist assessment practice 
  • Conduct assessments using recognised assessment materials, professional      observations and background information gathered from relevant sources 
  • Produce written reports within approximately three working days of the assessment      unless otherwise agreed 
  • Send completed reports directly to the fee-paying client via email 
  • Maintain confidentiality and data protection in accordance with UK legislation and      professional obligations 
  • Explain findings and recommendations clearly and professionally 
  • Maintain professional boundaries and safeguarding standards throughout all work undertaken 


Diagnostic decisions and recommendations are made using professional judgement based upon assessment results, background information, observations and the evidence available at the time of assessment.

  

4. Client Responsibilities

The client agrees to:


  • Provide accurate and complete background information relevant to the commissioned service 
  • Disclose any significant medical, educational, safeguarding or additional needs      information that may affect assessment or support 
  • Ensure that consent is obtained from all relevant parties where appropriate 
  • Attend appointments punctually and provide reasonable notice if cancellation or      rescheduling is required 
  • Ensure that a responsible adult remains onsite, nearby and contactable during      assessments involving children or vulnerable individuals 
  • Make payment in accordance with these Terms and Conditions 


Where one parent, carer or individual with parental responsibility provides consent or signs documentation relating to an assessment, Dyslexia Coventry will reasonably assume that all individuals with parental responsibility have been appropriately consulted and consent to the assessment proceeding.


Where inaccurate, incomplete or misleading information is provided, Dyslexia Coventry reserves the right to amend recommendations or withdraw services if appropriate.

  

5. Fees and Payment

For privately commissioned assessments or services, a non-refundable deposit of 50% of the agreed fee is required at the time of booking in order to secure the appointment date and time.


The remaining balance must be paid no later than three working days prior to the assessment or service date unless otherwise agreed in writing.


For schools, colleges, local authorities, adult education providers or other organisations commissioning services, invoices may be issued following completion of the work unless alternative arrangements have been agreed.


Reports will not ordinarily be released until full payment has been received.


Payment may be made via:

  • Bank transfer 
  • PayPal      
  • Cash 


Payments made via PayPal may incur an additional administration charge to cover third-party transaction fees.

Invoices not settled within the stated payment period may be subject to reasonable recovery action and statutory interest where applicable under UK law.

  

6. Cancelling and Rescheduling

Under the Consumer Contracts Regulations 2013, clients booking services remotely may be entitled to a 14-day cooling-off period.


However, by confirming a booking and requesting that assessment preparation, administration or professional work begins before the end of the cooling-off period, the client acknowledges that charges may still apply for work already completed should the booking later be cancelled.


Cancellation requests must be made in writing by email.


Where cancellation is made:


  • The initial 50% deposit paid at the time of booking is non-refundable, as it secures the appointment date, administration and reserved specialist assessment time. 


  • Where cancellation is made more than 14 days before the appointment date, any      additional fees paid beyond the deposit will normally be refunded. 


  • Where cancellation is made within 14 days of the appointment date, additional charges may apply due to assessment preparation, administration and the likelihood that the appointment cannot be reallocated. 


  • Within 5 working days of the appointment date, the full fee may become payable. 


  • Failure to attend an appointment without notice may result in the full fee being charged. 


  • Once an assessment or commissioned service has commenced, no full or partial      refund will be issued.


Requests to rearrange appointments made more than 14 days before the scheduled appointment date will usually be accommodated without additional charge where reasonably possible.


Dyslexia Coventry reserves the right to rearrange or cancel appointments due to illness, safeguarding concerns, severe weather, unforeseen circumstances or other circumstances beyond reasonable control. In such circumstances, alternative arrangements or refunds will be discussed.

Late arrival may reduce the available assessment time. Sessions will ordinarily finish at the agreed time.

  

7. Confidentiality and Data Protection

Dyslexia Coventry is committed to handling personal information confidentially and in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018.

Information collected may include:


  • Personal and contact details 
  • Educational and medical background information 
  • Assessment data and professional observations 
  • Reports and recommendations 
  • Communication records relevant to the commissioned service 


Information will only be shared with third parties where:

  • Explicit consent has been provided by the fee-paying client or adult learner 
  • There is a legal or safeguarding obligation to disclose information 
  • Disclosure is required for professional regulation, APC renewal, quality assurance or      moderation purposes 


Where reports are used for APC renewal, moderation or quality assurance purposes, identifying information will be anonymised wherever possible.


Assessment records and reports will be securely retained in accordance with legal, safeguarding and professional requirements.


For children and young people, records may be retained until the learner reaches 25 years of age.

Adult records are ordinarily retained for a minimum of seven years from the date of assessment.


Clients may request access to their personal data in accordance with applicable legislation.

  

8. Safeguarding and Professional Boundaries

Dyslexia Coventry is committed to maintaining appropriate safeguarding and professional standards at all times.


Assessments involving children or vulnerable individuals require a responsible adult to remain onsite, nearby and contactable throughout the appointment.


Services are educational in nature and do not constitute medical, psychiatric or clinical diagnosis.

Dyslexia Coventry reserves the right to end or decline services where there are safeguarding concerns, abusive behaviour, inappropriate conduct or circumstances that compromise professional safety, ethical practice or wellbeing.

  

9. Diagnostic Decision

Any diagnostic opinion provided within a report represents Sarah Grainger’s professional opinion based upon:


  • Standardised assessment data 
  • Background information provided 
  • Professional observations 
  • Relevant educational history 
  • The evidence available at the time of assessment 


A diagnosis of dyslexia or any other specific learning difficulty cannot be guaranteed prior to assessment.


Recommendations within reports reflect professional judgement and are intended to support educational, workplace or daily functioning.


Final decisions regarding examination access arrangements, workplace adjustments, funding applications or support provision remain the responsibility of the relevant educational institution, employer, awarding body or external organisation.

  

10. Use of Reports

Reports produced by Dyslexia Coventry remain the intellectual property and copyright of Sarah Grainger and Dyslexia Coventry.


Reports are provided solely for the use of the named client and for the purpose for which they were commissioned.


Reports are sent directly to the fee-paying client only. Dyslexia Coventry does not distribute reports directly to schools, colleges, universities, employers, GP services or other organisations on behalf of clients unless legally required to do so.


Clients may choose to share reports with:

  • Schools, colleges or universities 
  • Examination centres 
  • Employers or workplace support services 
  • GP services or healthcare professionals 
  • Tutors or specialist support providers 
  • Other relevant professionals 


Reports must not be altered, edited or reproduced in part without written permission.


Dyslexia Coventry accepts no responsibility for decisions made by third parties based upon report findings or recommendations.

  

11. Limitation of Liability

Dyslexia Coventry will not be liable for any indirect, consequential or financial loss arising from the use of services or reports provided.


All services are delivered using reasonable professional skill, care and judgement. However, educational outcomes, examination results, institutional decisions and workplace outcomes cannot be guaranteed.


Nothing within these Terms and Conditions excludes liability where exclusion would be unlawful under UK legislation.

  

12. Complaints

If a client has concerns regarding any aspect of the service provided, they are encouraged to raise these directly with Sarah Grainger in the first instance so that an informal resolution may be sought.

Formal complaints should be submitted in writing within a reasonable timeframe following the issue arising.


Complaints will be acknowledged and responded to within a reasonable period.


Where concerns cannot be resolved internally, clients may seek further guidance through relevant professional bodies, including the British Dyslexia Association or other applicable professional regulatory routes.

  

13. Review of Terms and Conditions

Dyslexia Coventry reserves the right to amend or update these Terms and Conditions periodically.

The most current version will be available via the Dyslexia Coventry website.


Continued use of services following any updates constitutes acceptance of the revised Terms and Conditions.
